About Me

I enjoy the functional application of medicine; it is extremely satisfying to help patients to regain the motor function that was once diminished. Life tasks such as walking and going to work, once lost to chronic pain, can be regained. My goal is to increase my patients' quality of life by correcting their problems and alleviating their pain so they can return to activities they enjoy.

My philosophy of care centers on performing responsible spinal surgery for appropriate candidates. I tailor the surgery to each patient's particular condition and needs. I use the smallest incision and most minimally invasive approach to resolve the patient's issue, because this approach allows for faster recovery and return to the activities of daily living.

A lifelong Floridian, Dr. McClellan attended The Benjamin School in North Palm Beach, where his interest in athletics grew as a high school football and soccer player. While attending Ohio Wesleyan University on an athletic grant for football, he advanced his knowledge of athletics as a pre-physical therapy major. When an injury permanently sidelined him, he returned to Florida to work as an exercise coach. After five years helping clients achieve major health changes, Dr. McClellan’s interest in a naturalistic lifestyle led him to chiropractic care. After just two weeks as a patient, he committed to pursue his doctorate of chiropractic at Life University in Atlanta, Georgia. Dr. McClellan graduated Cum Laude in 2009, returning to Florida to practice in his hometown.

Dr. McClellan is a specialist in treating disc herniations, spinal stenosis, and arthritic conditions of the spine. His approach has helped many patients who’d already failed with physical therapy, chiropractic, injections, medications, and even surgery. He developed the Altus Health Group with the philosophy of targeting the underlying reasons behind patients’ pain and providing an array of medical services to effectively address these root causes. Dr. McClellan is passionate about helping patients and the latest options to advance successful care. He and his team remain at the cutting edge of medical and therapeutic advances.

Dr. Jon Graham, a board-certified neurosurgeon, specializes in cranial and spine surgery for the treatment of cervical, thoracic, and lumbar spinal disease. Dr. Graham has a special interest in minimally invasive techniques. He has participated in a multitude of research studies and lectures, writing numerous articles through peer-reviewed publications on spine surgery.

Dr. Kaplan is the Head Team Physician for the NFL Jacksonville Jaguars and has been a physician with the team for 16 years. He is also the head team physician for the Bolles School where he graduated from in 1995. Dr. Kaplan is a Team Physician for Jacksonville University and takes care of athletes from the majority of high schools in the local and regional area. In the past, Dr. Kaplan was the Team Physician for the Arena Football League Champion Jacksonville Sharks and the University of North Florida. He also is asked to cover as a physician for numerous athletic events hosted in Jacksonville throughout the year including the PGA championship at Sawgrass.

Dr. Kaplan graduated from Tulane University (New Orleans, LA) with a Bachelor of Science, majoring in cell and molecular biology and was distinguished as Summa Cum Laude with Honors, Phi Beta Kappa, and a member of numerous National Honor Societies. He attended New York University School of Medicine (New York, NY) where he graduated as a Doctor of Medicine and was distinguished in the Alpha Omega Alpha honor society.

Dr. Kaplan completed his residency in Orthopaedic Surgery at New York University Langone (New York, NY) where he was chosen as an Executive Chief Resident, contributed over twenty orthopaedic publications and earned several research awards. He completed a fellowship in orthopaedic sports medicine at the world-renowned Kerlan-Jobe Orthopaedic Clinic (Los Angeles, CA). During his fellowship, he assisted the head team physician in the orthopaedic care of the 2009 World Champion Los Angeles Lakers. In addition, he served as an assistant team physician for the Los Angeles Dodgers, Los Angeles Kings, Los Angeles Sparks, Anaheim Ducks, and Loyola Marymount University. Working among innovators in the field of sports medicine, Dr. Kaplan has been trained in cutting-edge arthroscopy of the shoulder, elbow, and knee. He has presented his research at numerous national meetings and is a fellow-member of the American Orthopaedic Society for Sports Medicine and the Arthroscopy Association of North America. Dr. Kaplan strives to be a leader in the field of sports medicine, arthroscopy and patient care in the Jacksonville community.

Dr. R. David Graham graduated from the University of South Florida Morsani College of Medicine. After his time at USF, Dr. Graham began his orthopedic surgery residency at Southern Illinois University. Dr. Graham finished his Hand, Upper Extremity & Microvascular Fellowship at Allegheny General Hospital in Pittsburgh, Pennsylvania.

Carl Freeman, MD specializes in the treatment of hip, shoulder, and knee disorders in patients of all ages. He is one of a few Hip Preservation Surgeons in the United States. Advanced hip procedures include hip arthroscopy (hip scope), anterior total hip replacement (including robot-assisted), and surgical dislocation of the hip. Advanced shoulder procedures include shoulder arthroscopy for rotator cuff and labral repair, instability surgery, shoulder replacement, reverse shoulder replacement, and latissimus transfer. In addition, Dr. Freeman’s practice includes sports medicine and knee surgery.  He is board certified by the American Board of Orthopaedic Surgeons. Dr. Freeman graduated from medical school at Georgetown University School of Medicine and performed a residency in orthopaedic surgery at UPMC Hamot. Afterward, he completed a fellowship in Surgical Preservation of the Hip at the prestigious Schulthess-Klinik (the Swiss Olympic Medical Center) in Zurich, Switzerland. In 2011, Dr. Freeman was the first recipient of the International Society for Hip Arthroscopy (ISHA) Traveling Fellowship. In 2015, Dr. Freeman was selected to be one of four surgeons to complete the Arthroscopy Association of North America (AANA) Traveling Fellowship, which involved training in the most advanced surgical practices in sports medicine. Dr. Freeman spent four years as an active duty orthopaedic surgeon in the US Navy, which brought him to Jacksonville- stationed at Naval Hospital Jacksonville. In 2012, he had the privilege of treating our service members in Afghanistan during Operation Enduring Freedom where he spent time in a forward surgical team and served in a tertiary combat trauma center. In addition to being an active clinician, Dr. Freeman has contributed to academic medicine as Assistant Professor of Surgery at the Herbert School of Medicine, Uniformed Services University of Health Science, Bethesda, MD, and has authored numerous scientific publications. After being stationed in Jacksonville for four years, he and his wife chose to make this city their home to raise their two children. They enjoy spending time outdoors, water sports on the St. Johns River, and traveling with their son and daughter.  Dr. Freeman is the team physician for Paxon High School. Dr. Freeman is on the Duval County Medical Society Board of Directors and is a delegate to the Florida Medical Association.

Jason Piraino, D.P.M., is a University of Florida associate professor of orthopaedic surgery and a podiatrist and board-certified foot and ankle surgeon at UF Health Jacksonville. He also serves as chief of the UF Division of Foot and Ankle Surgery - Jacksonville, as well as program director of the Podiatry Residency. He received his podiatric doctorate from the Temple University School of Podiatric Medicine in Philadelphia, Pennsylvania, and completed his residency in foot and ankle surgery at Temple University Hospital in Philadelphia.

Dr. Kyle J. Lewis is an experienced chiropractor committed to serving his local community. He graduated from Palmer College of Chiropractic-Florida after completing his Bachelor of Science in Psychology at the University of Central Florida.

Growing up in South Florida, Dr. Lewis discovered his passion for chiropractic care while attending The King’s Academy (’08) in West Palm Beach. Struggling with chronic migraines, he sought holistic alternatives to traditional medical approaches.

Dr. Lewis coined the philosophy of “Don’t chase the PAIN, chase the PROBLEM,” focusing on identifying the ROOT causes of each patient’s pain. His team offers a range of services, including physical therapy, interventional pain management, and chiropractic adjustments, to effectively address these underlying issues.

While Dr. Lewis’ expertise lies in sports and pediatric chiropractic care, he also treats various other conditions such as herniations, spinal trauma, and general everyday aches and pains.

Beyond his professional skills, Dr. Lewis embodies compassion, trustworthiness, and humility. He is proud to be a part of the Palm Beach County community and provides top-notch care for families.

Doc, along with his wife Allison, and their two daughters, Hadley, and Harper, currently reside in Jupiter, Florida.
